How it works

A healthcare concierge service streamlines the care experience with evaluation → customized planning → care delivery → ongoing support, ensuring patients and families receive high-quality, coordinated, and compassionate care without the usual confusion or delays of traditional systems.

    1. Initial Contact & Intake

    • Client Inquiry: The patient or family contacts the concierge service via phone, email, or referral.

    • Intake Assessment: A brief consultation gathers basic information—current health concerns, medical history, needs, preferences, and goals.

    • Service Explanation: The client is informed of available services, pricing, and how the personalized care model works.


    2. Comprehensive Evaluation

    • In-Person or Virtual Assessment: A registered nurse (RN), case manager, or clinician conducts a thorough health assessment.

    • Medical & Social History Review: Includes review of diagnoses, medications, treatments, lifestyle, home environment, and support systems.

    • Needs Analysis: Determines level of care, coordination requirements, and personal goals (e.g., aging in place, post-op recovery, chronic disease management).

    3. Custom Care Plan Development

    • Collaborative Planning: A care coordinator develops a personalized weekly care plan based on the evaluation.

    • Multidisciplinary Input: The plan may include input from physicians, therapists, nurses, and wellness professionals.

    • Care Plan Components:

      • Nursing/medical oversight

      • Appointment scheduling & transportation

      • Medication management

      • Therapy or specialist referrals

      • Home care or companionship

      • Nutrition and lifestyle guidance

    • Family Involvement: Family members or primary caregivers are included when appropriate.


    4. Start of Care Journey

    • Care Kickoff: The care plan is reviewed and approved by the client, then officially begins.

    • Care Concierge Assigned: A dedicated care navigator or concierge becomes the single point of contact.

    • Provider Coordination: The concierge arranges initial appointments, referrals, and care team introductions.

    • Technology Setup (if applicable): The patient may be onboarded with apps, telehealth tools, or patient portals for communication and tracking.


    5. Ongoing Care & Weekly Management

    • Weekly Check-ins: The concierge contacts the client regularly to monitor progress, schedule appointments, and adapt the care plan.

    • Caregiver Support: If in-home care is included, weekly visits are tracked and reported.

    • Issue Escalation: Any health changes or urgent needs are addressed immediately through nurse triage or care escalation protocols.


    6. Real-Time Communication & Advocacy

    • Medical Navigation: The concierge communicates with physicians, specialists, and insurance providers on behalf of the client.

    • Documentation & Coordination: All records are centralized and shared securely across the care team.

    • Advocacy: The concierge advocates for the patient in medical decisions, hospital discharges, and insurance approvals.


    7. Progress Evaluation & Plan Adjustment

    • Monthly Reviews: The care plan is reviewed at regular intervals to assess outcomes and adjust services as needed.

    • Client Feedback: Satisfaction and wellness are assessed to ensure alignment with the client's goals.

    • Upgrades or Transitions: As health needs change, additional support may be added, or discharge planning may begin if the goal is achieved (e.g., full recovery)
