Intake Assessment: A brief consultation gathers basic information—current health concerns, medical history, needs, preferences, and goals.
Service Explanation: The client is informed of available services, pricing, and how the personalized care model works.
In-Person or Virtual Assessment: A registered nurse (RN), case manager, or clinician conducts a thorough health assessment.
Medical & Social History Review: Includes review of diagnoses, medications, treatments, lifestyle, home environment, and support systems.
Needs Analysis: Determines level of care, coordination requirements, and personal goals (e.g., aging in place, post-op recovery, chronic disease management).
Collaborative Planning: A care coordinator develops a personalized weekly care plan based on the evaluation.
Multidisciplinary Input: The plan may include input from physicians, therapists, nurses, and wellness professionals.
